Možnosti využití distanční terapie a telerehabilitace v ergoterapii u pacientů po získaném poškození mozku – pilotní studie

Abstrakt (CZE)

Východisko
Studie se zaměřuje na pacienty, kteří byli propuštěni z lůžkové rehabilitační péče do domácího prostředí, ale v důsledku nedostupnosti či nemožnosti dopravy na ambulantní rehabilitaci nemají možnost navazující ergoterapie pro horní končetiny (HK). Situace je ještě více akcentovaná v oblasti ambulantní ergoterapie, která je minimálně dostupná ve všech regionech České republiky.
Cíl
Cílem studie bylo zhodnotit aplikaci distanční terapie (DT) u pacientů se získaným poškozením mozku se zaměřením na terapii HK a ověřit, zda dojde k udržení či významnému zlepšení jemné motoriky paretické HK.
Metody
Studie zahrnuje sedm pacientů v subakutním stadiu zařazených do distanční terapie v domácím prostředí (pět žen, průměrný věk 40 ± 15,66 let, čtyři s hemoragickou cévní mozkovou příhodou, tři levostranné hemiparézy, sedm dominantních pravých HK), kteří dokončili terapii pomocí senzoru Leap motion a softwaru Rehamza. Pacienti byli hodnoceni na začátku (T1) a po ukončení DT (T2) funkčními testy pro HK Motor Assessment Scale (MAS), testy koordinace a dexterity 9 Hole Peg Test (9HPT) a testem Box and Block (BBT), Motricity indexem (MI) pro svalovou sílu HK, modifikovanou Ashworthovou škálou (mAS) pro svalový tonus. Soběstačnost byla hodnocena indexem Barthelové (BI) a byla monitorována i změna kvality života první měsíc po propuštění z hospitalizace dotazníkem kvality života Short Form-36.
Výsledky

Výsledky motorických testů ukazují, že došlo ke statisticky významnému zlepšení jemné motoriky, koordinace a motorického tempa dle 9HPT (p = 0,05) a ke statisticky významnému zlepšení funkce HK dle MAS (p = 0,05). V testu BBT bylo pozorováno klinické zlepšení v průměru o 4,28 kostek. U všech sedmi pacientů došlo ke zvýšení svalové síly ruky na plný počet (33 b.) dle MI. Adherence všech pacientů k distanční ergoterapii byla 100 %.
Závěr

Výsledky studie ukazují, že pacienti s lehčím motorickým deficitem na HK po získaném poškození mozku mohou pozitivně profitovat z distanční ergoterapie.

Background
The study focuses on patients who were discharged from inpatient rehabilitation care to their home environment, but due to the unavailability or impossibility of transport to outpatient rehabilitation, the patients do not have the possibility of follow-up occupational therapy for the upper limbs. The situation is even more strongly emphasised in the field of outpatient occupational therapy, which is minimally available in all regions of the Czech Republic.
Aim

The aim of the study was to evaluate the application of distance therapy in patients with acquired brain injury with the therapy focusing on the upper limbs and to verify whether the fine motor skills of the paretic upper limb are maintained or significantly improved after distance occupational therapy.
Methods

The study included seven subacute patients enrolled in distance therapy at home (five females, mean age 40 ± 15.66 years, four haemorrhagic stroke, three left-sided hemiparesis, seven right handedness) who completed the Leap motion therapy with Rehamza software. The patients were evaluated at baseline (T1) and at the end of distance therapy (T2) by the Motor Assessment Scale (MAS) functional test, the 9 Hole Peg Test (9HPT) and Box and Block test (BBT) coordination and dexterity tests, the Motricity Index (MI) for the upper extremities for muscle strength, the modified Ashworth scale (mAS) for muscle tone, self-sufficiency was assessed by the Barthel Index (BI) and the change in the quality of life was monitored with the SF-36 questionnaire in the first month after discharge from hospitalisation.
Results

The results of the motor tests show that there was a statistically significant improvement in fine motor skills, coordination and motor pace according to 9HPT (p = 0.05) and a statistically significant improvement in the upper limb function according to MAS (p = 0.05). A clinical improvement of 4.28 cubes in average was observed in BBT. In all seven patients, the muscle strength of the hand increased according to MI to the full score (33 b.) The adherence of seven patients to distance occupational therapy was 100%.
Conclusion

The results of the study show that patients with a mild motor deficit on the upper limb after acquired brain damage can benefit positively from distance occupational therapy.

Abstrakt (CZE)

Telerehabilitace je v některých zemích běžnou součástí zdravotní péče již několik let. V České republice se do povědomí širší odborné veřejnosti dostala až ve spojitosti s pandemií COVID-19, kdy vznikla akutní potřeba zajistit péči nejen v oblasti klinické logopedie. Prudký rozvoj telerehabilitace v posledních dvou letech s sebou nese také rizika spojená s využíváním různých prostředků, které nesplňují požadavky na kybernetickou bezpečnost a ochranu dat pacienta. Předkládaný text se zabývá vyjasněním užívaných pojmů telehealth, telemedicína, telerehabilitace a jejích forem. Informuje odbornou veřejnost nejen o celosvětové potřebě vypracování Standardů telerehabilitační péče, ale také o legislativních požadavcích na bezpečnost, ochraně dat a technickém vývoji telerehabilitace v České republice a možnostech jejího dalšího využití a směřování.

Abstrakt (CZE)

Neuromuskulární elektrická stimulace je jako terapeutická metoda využívána již řadu let. Zařízením přímo vyvinutým pro elektrickou stimulaci svalů hlavy a krku je VitalStim. V České republice se jedná o poměrně novou terapeutickou techniku, v zahraničí je již známa přibližně od roku 2001. Od počátku je však její využití pro terapii u pacientů s dysfagií mezi odborníky velmi diskutováno. Ačkoliv již byla provedena řada studií, doposud nebyla efektivita tohoto terapeutického postupu jednoznačně prokázána. Předkládaný systematický přehled si klade za cíl analyzovat výsledky vybraných zahraničních studií zaměřených na tuto problematiku a poskytnout tak ucelený přehled o možném efektivním využití VitalStim terapie v klinické praxi.

Abstract (ENG)

Neuromuscular electrical stimulation has been used as a therapeutic method for many years. VitalStim is the device specifically developed for the electrical stimulation of the head and neck muscles. In the Czech Republic, this is a relatively new therapeutic technique. However, it has been used abroad since 2001. Its use for therapy in patients with dysphagia has been widely discussed among experts. Although a number of studies have been performed, the effectiveness of this therapeutic approach has not been clearly established. The presented systematic review aims to analyse the results of selected foreign studies focused on this issue and to provide a comprehensive overview of the possible effective use of VitalStim therapy in clinical practice.

Abstrakt (CZE)

Počet pacientů, u nichž je v důsledku cévní mozkové příhody nebo kraniotraumatu diagnostikována afázie, neustále narůstá. Ukazuje se však, že pro řadu pacientů je intenzivní ambulantní logopedická péče z různých důvodů nedostupná. Možné řešení neutěšené situace by mohla nabídnout telerehabilitace. V zahraničí je tato forma rehabilitace poměrně běžnou alternativou klasické ambulantní péče, v České republice se s tímto pojmem teprve začínáme seznamovat. Článek nabízí přehled současných možností synchronních a asynchronních telerehabilitačních systémů, které lze aplikovat při terapii afázie.

Abstract (ENG)

The number of patients diagnosed with aphasia after stroke or traumatic brain injury is constantly increasing. However, it appears that for many persons intensive outpatient speech therapy is unavailable for various reasons. Telerehabilitation could offer a possible solution of this dismal situation. Abroad, this form of rehabilitation is a relatively common alternative to traditional face-to-face therapy, while in the Czech Republic we just get acquainted with this term. The article offers an overview of current possibilities of synchronous and asynchronous telerehabilitation systems that can be applied in aphasia.

Abstrakt (CZE)

Pro zajištění následné péče o pacienty se získaným poškozením mozku je důležité garantovat specializovanou komplexní rehabilitační péči. Kromě fyzioterapie a ergoterapie je nutné zajistit také logopedickou, dysfagickou a psychologickou péči. Rehabilitační ústav Kladruby (RÚ) spustil pilotní komplexní neurorehabilitační program – kranioprogram, s garancí dostupnosti logopedické a psychologické péče na základě indikačních kritérií, v roce 2015. Na základě zkušenosti s neodhalenými tichými aspiracemi z primární péče byl také zaveden sekundární dysfagický screening (n = 124), který pomáhá identifikovat rizikové pacienty (n = 27). Na základě těchto zkušeností byl sestaven dysfagiologický tým. V současné době je kranioprogram v RÚ plně hrazený zdravotními pojišťovnami a cílem je rozšíření tohoto programu i do dalších rehabilitačních zařízení.

Abstract (ENG)

Specialized complex neurorehabilitation care is important part of the complex neurology care for survivors with acquired brain injury. In addition to physiotherapy and occupational therapy, it is necessary to provide a speech pathology and dysphagia care and as well psychological care. The Rehabilitation Centre Kladruby (RÚ) started a pilot complex neurorehabilitation program – Kranioprogram in 2015, with a guarantee of availability of speech and psychological care, based on the indicative criteria. A secondary dysphagia screening (n = 124) has also been introduced to help identify risk in patients with silent aspiration (n = 27) based on the experience of undetected silent aspiration. Based on these experiences, a dysphagia team was assembled. Currently, the Kranioprogram in the RÚ is fully funded by health insurance companies, with the aim of extending this programme to other rehabilitation facilities.

Východisko
Pomocí roboticky asistované terapie pro lokomoci se současným zapojením horních končetin lze pacienty po získaném poškození mozku lépe připravit na provádění personálních i instrumentálních běžných denních činností. Tímto typem terapie lze předejít riziku pádu, zamezit nadměrné dopomoci další osoby, zvýšit participaci jedince v běžných denních aktivitách a zároveň zvýšit i kvalitu života.
Soubor

Práce demonstruje muže ve věku 65 let po hemoragické cévní mozkové příhodě s projevem levostranné hemiparézy, který byl v subakutní fázi přijatý do intenzivního rehabilitačního programu Rehabilitačního ústavu Kladruby.
Metody

Reoambulator je přístroj propojující nácvik lokomoce se současným zapojením horních končetin a tréninkem kognitivních funkcí. Sledovaný proband byl měřen v čase T0 po příjmu do intenzivního rehabilitačního programu, T1 před zahájením roboticky asistované terapie pro lokomoci a v čase T2 po ukončení roboticky asistované terapie. Ve všech časech byly použity testy Berg Balance Skore, Time Up and Go, 6. minutový test, FIM – motorické subskore, Action Research Arm Test, Motor Activity Log, Box and Blocks test.
Výsledky

U pacienta došlo ke zlepšení mezi T1 a T2 v chůzových testech, dle 6 – ti minutového testu ze 150 metrů na 178 metrů, dle Time Up and Go došlo ke zrychlení z 28,45s na 22,45s. V oblasti funkce horní končetiny pacient provedl před terapií 21 činností a po aplikované terapii 27 z 29 činností dle Motor Aktivity Log. V Box and Blocks se pacient zlepšil z 16 na 27 kostek a v Action Research Arm Test z 27 na 35 bodů.
Závěr

Využití roboticky asistované terapie pro lokomoci se současným aktivním zapojením končetin z pohledu ergoterapeuta vede ke zvýšení soběstačnosti pacienta. Pacient je komplexně připravován na provádění běžných denních aktivit, neboť dochází k současnému tréninku motorických, senzorických i kognitivních funkcí.

Basis
Via robotic assisted therapy for locomotion with simultaneous connection of upper limbs can patients after acquired brain impairment get better prepared for personal and instrumental activities of daily living. With this type of therapy patients may precede the risk of falls, prevent the excessive support of another person and increase both the participation of an individual in common daily activities and the quality of life.
Group

This work presents a man, aged 64, after haemorrhagic brain stroke with the manifestation of left hemiparesis, who entered the intensive rehabilitation programme in Kladruby Rehabilitation Institute in his subacute phase.
Methods

Reoambulator is a machine supporting the locomotion drill with simultaneous connection of upper limbs and training of cognitive functions. Presented proband was measured in the time T0 after entering the intensive rehabilitation programme, the T1 time before the start of robotic assisted therapy for locomotion and in the T2 time after finishing the robotic assisted therapy. Berg Balance Test, Time Up and Go, 6-minute walk test, FIM – motor score, Action Research Arm Test, Motor Activity Log, Box and Blocks test were used in all measured times.
Results

Between T1 and T2, the improvement in gait tests was observed, in 6-minute walk test from 150 meters to 178 meters, in Time Up and Go from 28,45s to 22,45s. In upper limb function tests, the patient performed 21 activities before the therapy and 27 from 29 activities after the therapy in Motor Activity Log test. In Box and Blocks test improvement from 16 to 27 cubes was observed and in Action Research Arm Test the patient improved from 27 to 35 points.
Conclusion

Use of robotic assisted therapy for locomotion with simultaneous connection of upper limbs from the view of an occupational therapist leads to increased self-sufficiency of a patient. Patient is comprehensively prepared for common daily activities performance, since we can observe a simultaneous training of motor, sensory and cognitive functions.

Porovnání časové náročnosti, cenové dostupnosti a reliability testů jemné motoriky pro pacienty po cévní mozkové příhodě z pohledu ergoterapie

Východisko
K hodnocení jemné motoriky pro pacienty po cévní mozkové příhodě existuje mnoho testů. V klinické praxi se ergoterapeuti musejí rozhodovat, který z testových nástrojů je pro ně nejvýhodnější. Často se rozhodnutí zakládá na psychometrických parametrech, ceně a délce administrace testu.
Cíl

Porovnat testy jemné motoriky z hlediska časové náročnosti, cenové dostupnosti a hodnoty test-retest reliability.
Metody

V databázi PubMed byly vyhledávány studie, které kriticky hodnotí nejpoužívanější testy horní končetiny pro pacienty po cévní mozkové příhodě. Byly vyhledány systematické přehledové studie v anglickém jazyku v rozmezí od roku 2013 do roku 2017.
Výsledky

Nejvyšší hodnotu test-retest reliability má Action Research Arm Test, Motor Assessment Scale a Chedoke McMaster Stroke Assessment s hodnotou 0,98. Nejkratší čas potřebný k administraci je u Box and Block Test (5 min.) a Nine – Hole Peg Test (10 min.). Z hlediska ceny jsou nejvýhodnější Chedoke Arm and Hand Activity Inventory, Fugl – Mayer Assessment, Motor Assessment Scale, Chedoke McMaster Stroke Assessment a Stroke Rehabilitation Assessment of Movement, které jsou zdarma.
Závěr

Výběr testu záleží na mnoha faktorech, jedním z nich by měly být psychometrické parametry nástroje. Většinou bývá zvolen nejméně časové náročný test, který ale nehodnotí více úchopů nebo funkci celé horní končetiny. Z hlediska cenové dostupnosti záleží na organizaci, která poskytuje finance k zakoupení testu, pořizovací cena velmi výrazně ovlivňuje rozhodování při výběru standardizovaného nástroje.

Basis
There are many tests to evaluate fine motor skills for stroke patients. In clinical practice, occupational therapists have to decide which outcome measure is the most suitable for them. The decision is often based on psychometric properties, cost, and duration of test administration.
Purpose

Compare fine motor outcome measures in terms of time, cost and test-retest reliability.
Methods

The PubMed database was searched for studies that critically evaluates the most common upper extremity tests for patients after stroke from 2013 to 2017.
Results

Action Research Arm Test, Motor Assessment Scale and Chedoke McMaster Stroke Assessment have the highest test-retest reliability (0,98). The shortest administration time take Box and Block Test (5min) a Nine – Hole Peg Test (10min). In terms of price, Chedoke Arm and Hand Activity Inventory, Fugl – Mayer Assessment, Motor Assessment Scale, Chedoke McMaster Stroke Assessment and Stroke Rehabilitation Assessment of Movement are free.
Conclusions

The choice of test depends on many factors, one of them should be the psychometric parameters of the outcome measure. Usually, at least a time-consuming test is chosen, but it does not evaluate multiple grips or the function of the entire upper limb. In terms of price, it is up to the organization that provides the finance to purchase the test, the cost of it greatly influences the decision to choose a standardized tool.

Mobile biofeedback low cost therapy system for home, outpatient and institutional rehabilitation care

Abstract (ENG)

Background and aims
The aim of the study was to verify usability of mobile biofeedback therapeutic system Homebalance in group-based therapy of patients with a diagnosis of vertebral algic syndrome as a substitute for individual therapy, to evaluate therapeutic effect of this type of group-based intervention and to evaluate effect on reduction of manpower requirements.
Methods

The study included patients with a diagnosis of vertebral algic syndrome in the subacute and chronic phases, who were hospitalized in Rehabilitation center Kladruby. 73 patients were included in the study. All patients received conventional individual physiotherapy (1:1 ratio) for 4 times per week and once a week they received group balance therapy with Homebalance, a system consisting of Nintendo Wii Balance Board and a tablet with developed therapeutic software for visual feedback, under a physiotherapist’s supervision (1:7 ratio). The whole program lasted for 6 weeks. This group balance therapy consisted of testing and reference exercises using Homebalance (3 min), therapy with Homebalance (5 min) and independent exercise (15 min).
Results

The design of therapy was accepted across the patient group without occurrence of any undesirable side effects. A more significant improvement was achieved by patients who performed poorly at the initial examination and had a scoring record in the therapeutic game which was higher than the standard for a healthy population. There was an improvement of 11 seconds (p=0.0002) in patients who practiced for 3-6 weeks. Stability examinations showed improvement in laterolateral symmetry. A more significant improvement in posture was detected when visual feedback was given. Notable decrease of 86 % in manpower requirements was observed as a result of employing only one therapist in group-therapy training units.
Conclusions

Mobile biofeedback system Homebalance is easily usable and effective tool for group therapy. To make further recommendations and to set up therapeutic planning, it is necessary to carry out more detailed study with additional objective therapeutic trials as well as to provide larger patient population for subsequent data analysis.
